My Health for Life Program

NEXT CLASS BEGINS APRIL 2019!

What is My Health for Life?

The ‘My Health for Life’ program has been developed by some of our leading health foundations including Diabetes Queensland, The Stroke Foundation, The Heart Foundation, QAIHC, the Ethnic Communities Council of Queensland, and Primary Health Networks (PHN) Queensland.  It’s goal is to educate and support Queenslanders to take  control of their health.

What does the course look like and when is it?

The course takes place over 6 sessions – one individual session, 4 fortnightly session and then one follow up session three months later.   The program is designed to give people at risk of heart disease and diabetes the tools to make significant changes in their life, to get your health back on track, and lessen your risk of disease progression.

Our role is to help you make healthy choices, to motivate you, to help you identify barriers to change and assist you past them, to set realistic goals and an action plan.  We will do this in a small group environment, giving you the chance to make personal connections and form a supportive community with others who are also in the same position.  Most of all, our passion is to support and empower you to take care of your health.

Do you qualify for this free course?

My health for life has been specifically designed for people who are at high risk of chronic disease. You are eligible for the program if you are:

  • 45 years and over and have been identified by a health professional as being at high risk of type 2 diabetes, heart disease and stroke; or
  • 18 years and over and of Aboriginal or a Torres Strait Islander descent and have been identified by a health professional as being at high risk of type 2 diabetes, heart disease or stroke; or
  • 18 years and over with pre-existing conditions (previous history of gestational diabetes, or have been diagnosed with pre-diabetes, high blood pressure or high cholesterol).

What do I do next?

There is a risk assessment on the website which is very easy and quick to complete.  If the assessment shows you are at high risk, then someone from My Health for Life will call you to discuss.  If you want to guarantee you get one of the limited spots available you should call our clinic prior to this to book your initial appointment.  These appointments have to be before the start date of the group sessions.  Our next group session starts on Wednesday 17th of April, 2019 at 2pm at our Waterford West clinic.  

As our exercise physiology team are sought after, please don’t delay booking or we may be unable to complete your initial assessment before the group class date.
